adj-na indicates that the word is a 形容動詞, not that it typically takes な. A 形容動詞 doesn't stop being adjectival when it's used predicatively. There are quite a few 形容動詞 that are rarely used attributively.
That being said, I hadn't noticed that sankoku (in contrast to the other kokugos) has 幸甚 as a noun only. Also, I might have been wrong to assume that 幸甚に is adverbial in 幸甚に存じる. Nouns can take に in 〜存じる expressions.
Noun glosses are less elegant but I think 幸甚 is in fact functioning as a noun in all the examples in the JEs and kokugos.
There's only one sense here. I don't think "obliged" is any less correct than the other glosses. It's synonymous with "grateful" within the contexts in which 幸甚 is actually used.

I like the glosses here, but I agree with Jim that placing [adj-na] at the front gives the wrong idea about usage.  I imagine the goal of [adj-na,n] is to be consistent with adjective glosses, but really the glosses do not correspond to adj-na usage. 幸甚な母親 is not "an extremely grateful mother".  "幸甚な人"　doesn't even return google OR even a single book result.

So putting [adj-na] up front is in a way even more misleading.

As part of the big picture in thinking about this, is there any way you can imagine adding similar glosses (grateful, obliged, happy) to 幸い?  They'd surely make a bigger impact there.

I still think there might be something to the idea of a dedicated [id] sense. The other problem here is that "幸甚" does not literally(or even figuratively) mean "obliged", and right now it kind-of looks like it does... Highlighting that the glosses are idiomatic might help with that.  I realize this is not something that is currently done.

My main concern with having "adj-na" prominent is that implies the use of 幸甚な, which is actually quite rare.

adj-na is correct. It's a 形容動詞 that's only used predicatively or adverbially. 
I don't think "usu. as ～です" is a helpful note, and 幸甚に存じます only accounts for a little over 10% of usage. It's difficult to explain how the word is used without cluttering the entry. I think a simple "usu. in letters" note is sufficient.

I don't see any kokugos that list anything besides 占い.  But we already had "卜" alone, and looking at furigana.info 占 can appear without い in much the same way.  

sanoku gives a reading [2] that isn't even in daijs.  It's unambiguous:

yourei for 占者(90 examples). rare furigana, but where it appears, it's about 2-to-1 for うらないしゃ over せんじゃ.  せんしゃ never appears.

Can't speak to historical usage, but modern use seems to clearly prefer せんじゃ over せんしゃ (the latter being nonexistent, even though the old kokugos seem to feel differently). 

うらないしゃ needs the full 占い者 form.  It really feels like うらないしゃ should come first as a reading, even though daijs doesn't give it explicitly.  This is probably one of those [col]/[form] kunyomi/onyomi splits we've put into one entry.

Just searching for "占者" alone, this is what I encountered(せんじゃ):

うらないじゃ(ja) was noted previously as only in nikk.  That would be a very unusual pronunciation (there are 0 other occurrences of verb-form-い者 as じゃ, and only a few nouns that end in いじゃ.  At best, most likely archaic.  For me, this falls into the "more harm than good" category to be visible.　There might be some book results from the 1800s, or I'd just drop it entirely.

Windows 11 IME cannot complete any of these but うらないしゃ.

ngram counts for furigana forms are useless for determining frequency, because せんしゃ and せんじゃ have more common, unrelated uses (戦車, 選者). The terms are too rare to search ngrams with disambiguating collocations.

Jim agreed with my proposal to change all [geom] to [math] and it was done for many entries but nobody finished the job. I was going to do that for over a year but somehow never gotten around to do it.

sankoku does not list this as [n] at all, and specifically gives 激甚さ as a noun form.  One way or the other, primarily as an adjective, so glosses should be adjectives I would think.

Is there a source for sense 2? The correct reading appears to be かくがた (which we have as a separate entry).
If we move that sense to かくがた, I think this entry can be dropped. 角形/かくけい does not mean "polygon"; it just appears in the names of polygons.
