Not sure on this... do we order on frequency?  It has become apparent to me that the frequency of this term is driven by "model", not "copybook". All the kokugos and JEs have an entry for 甚く/いたく. I don't think it should be dropped.　Also, I don't think this いたく is just the adverbial form of sense 3 on our 痛い entry. They're defined differently in the kokugos and the 痛い sense is archaic.
I think this is a better approach. I suggest dropping sense 3 of the 痛い entry.

I suggest dropping this entry altogether and just having the rare/uk sense 3 of 痛い/甚い. 痛く almost always means "painfully" and if it's found in the wild, this entry will be matched. None of the EJs I checked gloss "exceedingly" as いたく.

Looking at ngrams and yourei, the usual use is a variant of コップ酒を飲む. That is, "served in a cup" is more informative than "drinking from a cup".  This is also the first entry in daijs. This is definitely *not* [vs], so even though daijs also indicates drinking, I'm not sure how this would be done grammatically. 

Google image search result confirms that コップ酒 is not カップ酒.  daijs also agrees.

The "sake sold in a cup rather than a bottle" looks like an attempt at "カップ酒", and is not the same as [1]. It looks to essentially be the result of a spelling error/confusion. 

Potential resolutions:
1. Just simplify the gloss, as I've done.

2. Add the xref here, just in case. (not usually done for spelling errors, but コップ カップ is an endless source of confusion (for me at least)).

3. Add a gloss [2]:

I can find a few real hits but I really doubt it's an established term, unlike 自家醸造.

I don't inherently object to [rare] entries, but the "fix" to this one is an xref to 自家醸造, which kind of highlights that maybe this shouldn't be here in the first place...

This was added in 2012 with "Lots of results on google"(I suspect from 自家醸造). This was later downgraded to [rare], with a comment about "300 hits" (where?)

Is there an actual kokugo ref for this? Not on kotobank.

Searching for "自家醸", 自家醸造 is all I get (which we also have, and is quite common).

And this term would be self-evident as A+B in any case.

Sometimes "just a bird", but usually represented with the body of a bird and head/arms of a human. jitenon.jp shares this view, as does daijs. (i.e. the Japanese tradition)

***Realized がら落ち is already in JMdict.　The origin of the term as coming from ナイアガラ also seems inaccurate, as a separate finance source (posted at the bottom of this comment) lists the origin as coming from ガラガラ (1003290). It sounds probable to me that this is the actual origin and the Niagara Falls association came later.

That being said, ガラ on its own in Katakana does show up quite a bit, as well as the verbified ガラる. Perhaps a separate entry may be warranted?

Added second definition: ガラ to mean "massive drop".

This term comes from the last two characters of ナイアガラ and is most commonly used to describe a steep decline in the value of a stock or cryptocurrency, likening it to Niagara Falls.

A verb version "ガラる" exists as well, also mentioned in two of the sources listed.

Dialectical, but seems it's used in different regions with other possible meanings. The context I saw it used in was at sentence-end by Kansai-ben speakers. In a previous entry where we couldn't nail the region down, we simply tagged it colloquial.
