I very much doubt that this entry in its current state could cause offence and/or confusion. There are only two senses, and this is the preferred term when referring to milking of an animal. It would look odd leading with "breast pumping".

Normally you order senses based on how common they are, correct? Doing it the other way here without a good reason risks alienating/offending users who happen upon the word as it is most commonly used. suddenly seeing "milking a cow or goat" in the context of human breastfeeding can offend people, but the opposite wouldn't.

I'd leave it as-is. It's not really a distinct sense in that breast pumping is another form of milking. They could be merged.

I don't think that addition helped and the new gloss doesn't seem to fit.

Sorry, I hadn't twigged you proposed dropping 針眼 as well as みず. As it's in all the kokugos I think it's best in its own "arch" entry.

The entry is much cleaner and easier to read without it. If the inclusion of an archaic form introduces a lot of restr tags, I think it should be split out into a separate arch entry or dropped entirely (my preferred option).

In modern usage, "gaming" almost always refers to video games. I don't think we should expand the scope of the field tag.
