Well, the point you are making is plainly wrong, "-chan" is clearly among the suffixes that "yobisute" indicates you're not using. Daijisen says "「君」「様」「さん」_など_の敬称" (my emphasis). Koj mentions 殿・様・さん・君. That doesn't mean we need to include "殿" or さま here, it's not an exhaustive list.
The wording you're suggesting doesn't make sense to me either, "even '-sama'" seems to imply that "-sama" would be the least polite of the suffixes listed, when it's quite the opposite.

"-chan" is totally out of concern of this context.  Only whether addressing someone by "-san", "-sama", "-kun" OR NOT is the context of "yobisute".  That is my point.

Robin's suggestion has been there for a while and no one has responded. I think it's a good way to go. The "koyomi" romaji version seems to get little or no use - most WWW hits are for a brand of shochu.
I'll pull this back to one sense, drop the koyomi gloss, and unless there are objections I'll edit the glosses of the ~20 entries with "koyomi" in them.

Small change, I think "focus" is a better gloss for the meaning I've suggested. "unable to focus on work" vs. "unable to concentrate on work", this is mostly stylistic, I think, but google counts seem to roughly support this intuition.
