I don't think the wa-ei's are really saying it's really used for describing crosiers, I think they're doing a "this is close enough" type of translation that the frequently do (that I'm not really a fan of). Tellingly, Kenkyusha's wa-ei doesn't actually suggest "錫杖" for "crosier" but instead has 牧杖(ぼくじよう) and 司教杖.

Robin's suggestion has been there for a while and no one has responded. I think it's a good way to go. The "koyomi" romaji version seems to get little or no use - most WWW hits are for a brand of shochu.
I'll pull this back to one sense, drop the koyomi gloss, and unless there are objections I'll edit the glosses of the ~20 entries with "koyomi" in them.

A "coming to" sense for 来 was proposed in 2020 but I argued against it as it's a kanwa meaning, not a prefix. My position hasn't changed. 
With the obvious exception of 来日, 来[country] terms are quite uncommon, as Japanese people would usually talk about *going* to those countries, not *coming* to them. I don't think we need to add any others.

You only get 337 hits for 病院 if you click to the last page of results. This is the number of results that Google has retrieved (and it's always a small number). It's true that Google hits aren't a very reliable metric but a low hit count of the first page is usually a good indication that the queried term is uncommon.
Japanese is absolutely full of puns. I don't think they belong in jmdict unless they're especially common or in a major ref. I don't think this one is anywhere near common enough.
