Where's the evidence of this being used in reference to city dwellers in other countries?

I think this latest revision by Robin makes the entry misleading and less useful. Just because 都 most commonly refers to Tokyo doesn't mean it's not applied to other cities when used outside a Japanese context. The fact that most of the kokugos adopt this insular mentality should not be taken as gospel IMO.

With that note I'd leave it where it is. It's the first sense in the JEs (and GG5 only has 込む) and in the Tanaka sentences 40 are using 混む and 8 are using 込む but they ALL mean crowded.

If move it down to sense 3, it still comes 
before the suffixes. I think it's really weird 
to lead with a meaning that's not at all 
common for 込む now that 混む has been split 
out.

I don't think sense 1 should be sandwiched between the suf senses (or moved to the bottom). How about a note?
