I'm not happy with a new sense arriving like that without a reference or case. I'm tempted to remove it and ask アノニマスさん to resubmit with justification.
That said, I suspect that "huh?" could well belong in sense 2, along with "erm".

As I understand it, the bureaucrats in the education ministry decided in 2000 that 搔 is the proper ("印刷標準") form of the kanji and 掻 is an acceptable simplified variant ("簡易慣用").

Practically every recently published kokugo from a variety of different publishers (daijirin, meikyo, iwakoku, etc.) uses these 印刷標準 kanji rather than the simplified forms.
  Comments:
Many entries will be much messier if we display both types of forms, so I think it's probably best to stick with one. Lately we've been hiding the "proper" forms and keeping the simplified forms visible. I think that's probably fine. If we want to switch to the 印刷標準 forms someday, it would be simple enough to write a script to swap them.

I agree [rK] probably isn't a good tag for these kanji forms, since they're quite a bit more common now than when the 2007 n-gram corpus was compiled. People are bound to encounter them frequently and wonder what they are. Maybe that information is better explained via a dedicated kanji dictionary rather than a vocabulary dictionary like JMdict.

I don't think 搔く rates as [rK]. Kanjipedia is a form of kanwa; not the sort of reference we'd use to justify word level [rK] tags. 搔 entered the JIS standards in JIS X 0212 as an 異体字 of 掻. Even after 20+ years of Unicode it's still rather rare. Wiktionary says it's a 旧字体, but I don't take that as authorative.

On reflection, I think the "oK" on 啞 is probably appropriate. 啞 is a very rare kanji. It was never in JIS X 0208, but got into JIS X 0212 and was included in JIS X 0213. Shibano's JIS 漢字辞典 simply says it's an old variant of 唖. "oK" really means out-of-date kanji, and this is certainly one. It's not in any of the other kokugo entries for 唖々/唖唖 AFAICT.

川の字
かわのじ
1. [exp,n]
《as 川の字で寝る, 川の字になって寝る, etc.》
▶ (formation of) three people sleeping side by side (esp. a child between two parents)
▶ [lit] "river" character

  Comments:
Yes, tricky. I think what you've done is fine in the circumstances.
FWIW I did see some cases of 川の字 being used as the label for the sexual activity, hence my suggestion of a sense for that.

  Comments:
the "エロ" search isn't evidence it means "threesome", I just wanted to demonstrate that it's not always 2 parents with the child in between, that it's wider than
that, that it can refer to any formation of 3 people sleeping next to each other.
"as 川の字で寝る" wasn't an example, it's the form it almost always appears as. all the results on the first page of google are variations 川の字で寝る or 川の字に寝る
I don't think "making a 川 pattern" is a great note. I don't quite understand what it means.

Maybe this? The note is not for examples.
