Many of my refs (Kanjipedia, gendai reikai, daijirin, meikyo) display the readings inline with this expression, so 蛙大海 is displayed as 蛙(かわず)大海(たいかい). That sort of setup makes the expression easier to parse. That option isn't available to us here, but we can use a comma to achieve the same sort of effect.

I wouldn't suggest that we start inserting commas everywhere they may be useful without regard for real-world usage. But the comma usage here is supported by some minor Japanese refs, and I think the utility of the comma is more important than the inconsistency with the major refs.

Should we really use commas for expressions where they aren't common? It's useful for breaking down the expression, but the implication is that this is the most common form.

The "故事成語を知る辞典" (linked below) seems to be the only ref that uses a comma. I have 8 other refs that don't use it (Daijr/s, koj, kanjipedia, etc.).

The comma here seems useful to me.

It's primarily used for food. 
All terms can be used in other contexts, e.g. I can say "I changed gear" without it actually being about a car.

I hadn't noticed that, but I still think we should ignore it, along with most other 〜日 terms. The major refs only have the 日〜 forms.
It isn't difficult to look up the characters individually. That's what we expect people to do with most two-country kanji terms. Only a small fraction of all the possible combinations are included in jmdict.

Kotobank's new interface uses 葡日 specifically meaning Portuguese-Japanese [dictionary], which is what prompted the inclusion of this entry.

It's not arbitrary. For these two-character terms, 日 usually refers to the country, and in standard usage 日 comes first (e.g. 日米関係, not 米日関係).
和 is used when referring to the Japanese language, and there's a semantic difference between 和伊 and 伊和 so it makes sense to have both. (A 和伊辞典 is not the same thing as a 伊和辞典.)
日 can also refer to the Japanese language (usually in the context of translation or interpretation), but with the exception of 英日, 中日 and 韓日, 〜日 terms are uncommon.

That seems arbitrary. We have both 伊和 and 和伊, for instance. No harm there.

OK, I'm axing this, and I've raised Robin's suggestion as an issue.

I think we should ignore this entirely. If we record it as "Sammatīya", it's only going to confuse people who encounter (and look up) the 三密 typo. If we include it as an iK form of 三密, we're no longer using the tag consistently.
三蜜 is not "irregular" in the sense that most people understand the term. Also, there are far more common misspellings that we would never include in jmdict, so I don't think "guiding users to the right word" takes precedence over all else.
I'm not opposed to redirecting users from 三蜜 to 三密; I just don't think 三蜜 should be displayed or described as an irregular form. To aid searches, maybe we should consider adding hidden kanji and readings fields to jmdict. That would put an end to a lot of the arguments over whether to include particularly quirky or obscure forms. We could stick common typos in there as well.

even googling "三蜜" 仏教, at least 90% of the results seem to be about 三密. I'm not opposed to having this as an entry if it is 
indeed correct, but I'd prefer not to have to lean on a single source, and looking at the search results, I'm just re-convinced 
that we should really be including 三蜜 as an iK in the 三密 entry. I don't think it's really relevant whether a Japanese native 
speaker would notice it was the wrong kanji if they looked at it closely, it's about guiding our users to the right word. In the 
absolute majority of cases somebody is using the word "三蜜", it's just a typo for 三密. That entry should be among the results 
shown for a query of that word.
