From this blog post (and the fact that it was introduced as an Osakan word on the popular TV show "Kenmmin show"), it seems it's not 
widely known outside of Kansai, so I think the vulgar sense should come first. 
(my 3 Kansai native informers are strictly speaking Osakans)
