I think it's "just" a quirky way of writing 縦と横, although as that page says it has a particular meaning in weaving. It might be grounds for having a 縦と横/たてとよこ/タテとヨコ entry. The Reverso matching site has them all meaning "vertical and horizontal", as you'd expect.

I'm OK having this as an entry (and prob. あの道 and この道) but I don't think the 2nd sense should be there. Either we do every single 
sense from 道, or we do just one, or we're being misleading. If it really must be kept, I think the note must go. What does "usual 
meaning" mean? I have my doubts it's actually "usual" as in "more common." But if it were, why isn't it sense 1? I think small 
inconsistencies like these makes the dictionary harder to use.

Probably time to call it a day on this one. I'm leaving it in largely because of its presence in the JEs. I'm also leaving in the "usual meaning" because I'm aware this is primarily a decoding/passive dictionary and there's a risk that learners might think sense 1 is the only meaning of the expression.
I see that GG5 also has この道 entry referencing the same sense of 道 (but not one for あの道.)  [Daijr has a この道 entry too; for the 1927 song "この道はいつか来た道".]

I've sat on the fence on this for a long time, as I can see both positions are valid. I think I'll leave it in:
- it is a Daijirin entry (although that just points to と言えば)
- the n-grams seem to be saying that 言えば gets a lot of use without the と, certainly in contrast with the other と言え... expressions.

We actually include the と for と言う expressions (See と言うと, と言うのは, と言うことは, と言うのも, etc.) All my refs (kokugos, JEs, grammar dictionaries) do the same. I think we can safely delete this. It doesn't serve any purpose as long as we have a と言えば entry.

Checking across other city names in Japan where katakana is used, it seems to range from rarish up to quite common (e.g. ヨコハマ, where it's the tyre/tire company). ヒロシマ and ナガサキ stand out though, and WWW searching always brings up the bombing. I think it needs to be mentioned somehow.
There are really 3 ways of handling this:
A. a separate ヒロシマ (and ナガサキ) entry with a gloss such as "Hiroshima (esp. when referring to the 1945 atomic bombing)";
B. adding the katakana to the main 広島 etc. entry as a "nokanji", with a note saying "ヒロシマ esp. when referring ...".
C. as for B, but with a second sense instead of the note.
I'd like to canvas views as to which is preferred. My preference is A, mainly because I think the city name entries should be even-handed and neutral. I like modern-day Hiroshima and while its unfortunate history is very evident, I don't think it it should be highlighted in the main dictionary entry like that when there's an alternative approach available.
