  Comments:
I thought "grace of God" more or less covered covered daijr's sense 2 (where you express gratitude for some situation or result that didn't involve someone else's help or efforts), but on reflection, there's nothing inherently religious about "おかげさまで無事に帰って参りました".
I'm not sure what the right approach is here. Probably best to drop the おかげさまで x-ref from the "grace (of God)" sense and instead just treat it as an extra-polite form of the equivalent おかげ sense (as in daijs).

This isn't in any of the JEs or meikyo/shinmeikai (and the kokugo examples are very old) but it's still used in Kansai.
I suggest handling this in the same way we did for 偏固.
I think ik is correct for えづく. It's still quite common. The first link notes: "「えずく」を「えづく」と表記しているのを見かけることがありますが、正しくは「えずく」と書きます".

The few entries about this are explanations saying that O型 was actually originally 0型. not in Japanese but in the original German. ゼロ型 can also refer to a number of different other things which becomes apparent if you google for "0型" -血液.
I don't think this can be called an established term meaning "type O blood" and having this as an entry can be misinterpreted. I wouldn't mind including 0型 as an iK in the O型 entry but it doesn't belong here as a separate entry.

the fact that this one particular phrasing is so much more common than several other perfectly acceptable ways of saying the same thing does 
make it a set phrase, to my mind. (I also feel 時が経つにつれて is a set phrase and wouldn't oppose including it as an entry)
