My first reaction was to disagree. Daijirin, GG5 and all the other JEs put the nausea sense first. Then I looked at the Tanaka examples and 4/5 were for the irritation one and the other is ambiguous. Then I looked at a sample of the non-dictionary WWW pages, and they indicate that the irritation sense is more common. I think what you have done is most likely right.

I'm not sure. We don't use the oK tag consistently. Because it's often difficult to tell whether a kanji form is old or just obscure, we tend to use it only for 旧字体.

I tried to close it with : They're non-joyo and don't appear in references. I'm comfortable with "oK".
That said, I guess "rK" is OK, but I feel "oK" is possibly better here.

御役御免 is in GG5 and meikyo (which is unusual for 御- terms). It's also not that rare. I think it should be visible.
Added sense.

I think this is just a typo, but... there's no such thing as a "vowel stem". Perhaps "verb stem" is meant? I'm not familiar enough with this process to say exactly what sort of formatives it affected.

It's in Koujien, which doesn't have it for むかつく. I don't know about interpolating okurigana - I prefer for something obscure like this to stick to the references.

I think the kanji should go, alternatively be added to むかつく
as well. not in daij/nikk any of the other dics and few hits
("嘔く" 934 results). also shouldn't the correct form be ”嘔き”?
