  Comments:
Iwanami's kokugo is the only one I know which does both 他自 and 自他. By the way, Iwakoku and shinsen kokugo jiten are also the only ones (that I know of) with の-adjective tags like we have.

I'm not saying we should change our policy, but I'll just note that iwakoku has 自他 on this word.

  Comments:
We always put vt first (the kokugos always put 自 first). The order doesn't indicate whether transitive or intransitive usage is more common.
I see that GG5 and meikyo have transitive examples.

Swapped [vi,vt]. That's it. Maybe usually not so important, or paid attention to, but [vi] so low that pedagogically I'd rather pretend it didn't exist. But oh well.

sankoku agrees 自他さ

ngrams show 130,000 strait suru uses, 911 straight transitive uses. Usual particle is に or と, not を.

を uses usually causative:
a mainichi を hit for させる "made me resent":
しかし、それよりもさらに私を憤慨させたのが何か分かるだろうか
