jawiki says, in it's Maiko entry: "東京など関東地方における「半玉」「おしゃく」とは別の文化起源であるため、区別されるものである。"
but the kokugo's don't say whether it's just Tokyo or elsewhere as well.
  Comments:
Sorry, what I meant is that "hangyoku" is the Tokyo term for "maiko", the Kansai equivalent. In English, "maiko" is the much more well-known 
word, though. (see books.google.com/ngrams) In Japanese too, actually:
