tried to find some usage examples but pretty much 50% of 
what I found were used in the wrong sense (i.e. the 
doctor coming to your house), while it wasn't clear 
whether it was at an office or the doctor's home in the 
rest.

宅診 doesn't seem to be a very common word and frequently 
misunderstood/used in the wrong way!

I guess "医者が自分の家で" can be taken to mean either the 
doctor's own home or their office. I mean the smallest 
clinics are sometimes both.

yes, i think it does mean the doctor's own house. but are we sure that the waeis have not confused meeting at the doctor's house with meeting at the doctor's office?  all my dictionaries suggest 医者の家 or similar

I think that "医者が自分の家で" possibly means at the *doctor's* own house.

are we sure this is "consultation at the office"?  kokugos define it as the doctor seeing the person at his own home ()医者が自分の家で患者を診察すること, etc.)
