sense 4 is a repeat of what we've already got, and i think our wording of sense 1 is vague enough to encompass sense 5 (plus kojien is the only dic that even mentions it)

with all these restrictions, and given that the first sense (and the reading itself) it's not very common, it's getting to the point that てんしん should be included as an 'also pronounced as'
