As far as I can tell, sense 1 is [vs] without exception.
I don't think there should be an separate "exciting" sense because the feeling described here is the same whether you're nervous or excited - it's about being on edge.
You wouldn't use はらはら to describe the excitement a child feels upon being told he's going to Disneyland.

I think the old "exciting; thrilling" sense is needed, either as part of sense 1 or in its own sense. 心配 can mean "suspense" and I think 成り行きを心配して気 needs to be seen in that context. 8 of the 11 Tanaka sentences using はらはら are about being excited; not necessarily anxious.
