<upd_detl>I don't think か知ら should be added to this entry, just as it's not in the kokugos, major JEs, etc. It's simply not used as a way of writing かしら. It gets highish n-grams counts and shows up in the yourei.jp site, but that's because of the aggregation of morphemes following parsing.
If we want to indicate its source we could have a note, as in Daijirin/Koj, or create a か知らぬ entry and xref to it. I'll suggest such a note, but I'm not sure it's needed.</upd_detl>
