<upd_detl>I noticed that どうかした is currently the main headword for entry 2407710 (by mere chance, etc.), but in reality it seems far more commonly used in the "is something wrong?" sense covered by this entry. It seems to appear in more forms than just the set phrases that 
previously made up this reading, including どうかしたの, どうかしたか, どうかしましたか, or sometimes just どうかした.

So, I think it makes sense to convert this entry covering the "is something up/wrong" meaning to simply どうかした and どうかしました, and make entry 2407710 just cover "どうかして".</upd_detl>

All of the sense 1 examples in GG5 and the Tanaka sentences are some variation on どうかしたの. Reverso has a few examples without の.
