<upd_detl>All of those refs back the original gloss, e.g. Koj: "小さなこども。幼児。", Daijr: "こども。しょうじ", GG5: "a 「young [little] child; an infant.". Many of the compounds where it is used as a modifier have it translated as "infantile" or "pediatric".
I think that although technically and legally it might mean up to 12 or 16, in practice it means quite young. The fact that JR East and JAL are using it for varying age ranges is interesting, but I suspect it's more that they need words to label their ticketing policies than a reflection of general uses.
Google images for 小児 show a lot of quite young kids.
I think it should revert to the original gloss, perhaps something like "child (usu. quite young, but poss. up to 12yo); infant".</upd_detl>

<upd_detl>i agree it does generally mean 'small child' or 'infant'.  i think the original entry was fine and i don't think we should specify an age range at all.  the word 'child' is also ambiguous in age range, so i think it works fine here</upd_detl>
