Again, I'm not an expert, but I think the ones that lack archaic verb equivalents in dictionaries are just neologisms (e.g., a 'new' [v5s] that hasn't been around long enough to have had a [v4s]), so I don't think there's an algorithmic way of doing it.

I think we can pass on the exceptions for the moment.  AFAIK, there aren't many and archaic Japanese isn't that much of a priority.  Eventually, I'd add (for example) [v4s] to all non-exceptional [v5s] entries.  Then I'd hide the [v4s] from the end-user by default in order to reduce clutter that 99.9% of users don't care about.  (Adding and then hiding by default is the same approach I've suggested for handling obsolete kanji and kana forms.)
