OK, so it means "when used with verb in the affirmative, it's especially common as どうも~しまう"
I get (now) what the note is trying to convey, but considering the sense is most commonly accompanied by a negated verb (daijr even makes a note of 
it: "(多く打ち消しの語を伴って)"), I think it's overly confusing. Out of the 10 example sentences for this sense in daij and GG5, only 2 are not 
with negated verbs, but neither is in the form of どうも~しまう(gg5 "どうもねえ" and daijr "どうも失敗ばかりする")

Tidied up PoS tags. Added [restr] tags for senses where あんまり doesn't apply.
Added sense (new sense 4) which corresponds to sense 2 in daijr/s. I noticed the Tanaka sentence "ひなたぼっこをしていると、あまりの気持ちよさに、ウトウトしてしまった" and felt that none of the existing senses adequately covered it.
Almost all the sentences are for sense 2 so they aren't affected by this amendment but there are 4 sentences (tagged as sense 3/4) that need updating.

I think your textbook has a typo. All the refreences are quite clear on the differences between 軽率 and 軽卒. That said, it is likely that some people are using 軽卒 when they mean 軽率 (typical 変換ミス). The question is whether to add 軽卒 to the 軽率 with an "iK" tag.

Just learned yesterday – asked someone what the hanging adds in rail cars were called, and got this answer, confirmed to be in use by search.

Might be a Kansai dialect term (Kyoto, reference is in Mie).

吊り広告 appears to be the more formal term as per WP, and 中吊り (which we have) is preferred by the dicts.
