My source for [2] and [3] is Kenkyusha's New Japanese-English Dictionary (5th Edition). An example of [2] is "2 部屋にキッチンとトイレ a kitchen and a toilet in addition to two rooms" and an example of [3] is "お礼にもらう receive sth as a [in] token of sb's gratitude". My source for [4] is The Handbook of Japanese Grammar by Harold G. Henderson (pages 203-204) which notes that this particle is used in classical Japanese.
