<upd_detl>I read the previous comments. The first sense is apparently not [uk].
Encountered as subtitles, kanji with furigana. 鞘 is not joyo, but unsheathing swords is not the usual domain of newspapers either. The wikipedia page uses the kanji exclusively. A cursory search through wikipedia showed the kanji used most of the time in all sword contexts. That movie title choice in 2012 may have been a stylistic choice.
I've confirmed [3] is [uk] at the updated xref.
A lot of サヤ in the wild are pods for green beans and edamame. I think [2] is at least *relatively* uncommon, if not rare. It's not the cap to a bic pen.
This product is called 鉛筆サヤ. (Was not easy to find...)
https://store.shopping.yahoo.co.jp/kawauchi/2rmn71555.html
キャップ seems far more common here. [2] seems almost [rare] to me, but hard to say. [4] I have no idea.</upd_detl>
